Supplier Quality Evaluation
Questionnaire

We changed the Supplier Quality Evaluation Questionnaire making it shorter and simplier. To fill it out, you will need the free Adobe Acrobat Reader. If you don't already have it, you can download it here:

Once you have the Acrobat Reader installed, follow these instructions:

1. Right click on this link and select "Save Link as. . ." from the pop-up menu.
2. Download the file then open the folder you have downloaded it to and double click on the file named, "FPG-046-2 Supplier Quality Evaluation Questionnaire.pdf" to open it in the Acrobat Reader.
3. Click on the "Fill and Sign" icon in the right margin.
4. Position the cursor over the field to be filled in, click on it once to set the cursor then click again and type in the information.
   Move the cursor over the next field and repeat until done.
5. Once you are finished, click "File" in the top menu then "Save."
6. Click this to send us your filled in questionnaire by attaching it to the E-mail along with the quality management certificate (if any) that you marked in the form.
